An innovative new service introduced by NHS Sandwell and West Birmingham (CCG) to improve the quality of end of life care for patients has been shortlisted for a national award.

The CCG has been shortlisted in the ‘Compassionate Patient Care’ category at the prestigious Health Service Journal (HSJ) Awards. This award is all about celebrating excellence in putting people first, engaging patients and families in their care, listening to views, and ensuring people are treated with care and compassion.

The HSJ Awards, to be held at the InterContinental O2 on 21 November 2018, are the leading awards in healthcare. The awards champion best practice and innovation to overcome the challenges that face the health sector.

NHS Sandwell and West Birmingham CCG launched the Connected Palliative Care (CPC) Service in May 2016 to provide an innovative and responsive 24/7 community based end of life care service to a diverse range of patients and their families. Delivered in partnership by NHS Sandwell and West Birmingham NHS Trust, the service includes a single point of access co-ordination hub, an urgent response team and Home from Home beds for the last few weeks of life.

Since launching, the CPC service has seen positive outcomes with 100% of patients and families that used the service saying they would be extremely likely to recommend the CPC service to people if they needed similar treatment.

Welcoming the award, Dr Nick Harding, Chair of NHS Sandwell and West Birmingham CCG praised the work done on the service:

"We are delighted to be shortlisted for this prestigious award which attracted a high number of entries from high performing health organisations across the country. Being shortlisted for this national honour is a clear demonstration of the innovation and partnership working that is being delivered to transform the quality of patient care in Sandwell and West Birmingham.

"End of life care is a key priority for the CCG. We are passionate about getting end of life care right for all our patients and also the experience for their families, friends and loved ones. The innovative CPC service aims to provide the highest levels of care as well as involving patients in the decision making of where they wish to receive palliative care and where they would prefer to die."
